Application Checklist for New Students

• Complete the online  Registration form (2026-2027)

• Pay the registration fee of $100 on us.mohid.co/md/mdrgn/iccl/masjid/online/donation/25  

• Academic report(s) from previous school 

• Copy of the child’s birth certificate

• Copy of child's immunization records

• Health information 

  • View required vaccinations 

  • Health evaluation form (linked below)

  • Vaccination record (to be filled out by physician)

• Print, sign, and return the tuition agreement 

• Homeschool notification form: select option B & write Islamic Community Center of Laurel Academy as the non-public school/homeschool umbrella. Make sure to send it to the address on the form.
