We are doing Assistive Technology Assessments during Distance Learning. The first step is to complete the Assistive Technology Referral Form (link below) and choose the Student Specific Request in the first question. The form is meant for a Case Manager or other School District Staff to complete. You are welcome to get input from parents/guardians, but they are not able to access the form.
Remember, DO NOT get consent first. After we receive the referral from you, we will review the provided information and then contact you to schedule an Evaluation Planning meeting with the parent or guardian. The Evaluation Planning Team (including AT Specialist) will determine if an assessment is appropriate and which assessment tools will provide the most useful information given the student's individual needs.
