Print, Sign, and Turn in the Parent Permission Form to your Guidance Counselor.
Missing school? Turn in Absence Permission Form to the Main Office.
Fill out and take the Medical Form with you to your job site.
Have your on-site mentor fill out the Evaluation Form & Hours Log.
Interview your on-site mentor. Using your experience and the interview as a guide, write a 2-3 page reflection, including at least two photos. Only one reflection paper is needed - if you do more than one experience, combine your reflection into one paper.
Turn in Evaluation Form, Hours Log, and Reflection Paper by the deadline! No late work accepted!
