  • Complete the Online Registration Application

  • In order to enroll, the enrolling parent or guardian MUST have the following proofs of residency documents in their name:

    • Valid Lease or Rental agreement with a parent as leaseholder and signed by both parties AND a GAS or ELECTRIC bill dated within 60 days with the enrolling parent's name and residence address.

 OR

• Property Tax Bill or Settlement Statement in parent’s name AND a GAS or ELECTRIC bill dated within 60 days with the enrolling parent's name and residence address.

OR 

• • • An affidavit of residency – affidavits of residency are obtained through Henry County Schools Welcome Center, 166 Holly Smith Dr. Mcdonough Ga. 30253. 770-957-6601.

• Additional enrollment documents include: 

  • Parent/Guardian photo identification

  • Proof of Legal Guardianship (if applicable)

    • This is required if the student is being enrolled by someone other than the birth parent listed on the birth certificate.

  • Copy of Birth certificate (proof of child's age)

  • Copy of Social Security Number (used for federal reporting ONLY- wavier option)

  • Georgia required health forms:

    • Georgia Certificate of Immunization (Form 3231 must be current)

    • Copy of Georgia Form 3300 (Eye, Ear, and Dental Screening)

    • ALL 6th grade students must have updated immunization records before entering middle school!!!

• Transfer students must provide the following information:

  • Proof of grade placement

  • Final report card from previous school

  • Name and address of previous school

The enrollment process MUST be complete and all required documentation MUST be uploaded or provided BEFORE a schedule is created for your child. 

• An appointment is needed to complete a withdrawal at least 24 hours from your child's last day at school.

• Same day withdrawal requests may not be processed.

• In order to withdraw, the enrolling parent or guardian MUST have the following documentation:

  • Parent/Guardian photo identification

• All fees (lunch, library, etc) MUST be paid and the student's Chromebook and charger MUST be returned before the withdrawal process is complete.

  • Not fulfilling this requirement may prevent
