Prevalent Medical Conditions

If your child has a medically diagnosed prevalent medical condition (anaphylaxis, asthma, diabetes, or epilepsy/seizure), please inform your child's teacher. The applicable forms need to be completed and submitted with the student's epi-pen, inhaler, or other medications by mid-August, prior to the start of the school year. As well, a quick meeting in person or telephone call with the Vice Principal will be required. These forms and medications will also be required for any school related trips and activities.
