Change of Address Form - 

If your family has moved or is planning to move it is important that you update your address as soon as possible.  Please provide the following documents to the Student Services Office:

• Completed Change of Address Form (link form)

• Complete, current lease or deed (signed by all parties)

• Full first page of a current utility bill (BGE, water/sewer, landline phone/internet or cable television)
