Improving home accessibility may include some minor modifications to support independence and make the participant’s home safer. Based on available funds, these minor modifications could include such things as: grab bars, handrails, modified door thresholds, swing-clear hinges, toilet support frames, bed support rails, or even things like lever type sink hardware, door hardware, or smoke detectors.
Home modification recommendations will be included in the HomeMAP report, and from this report the participant can prioritize any desired minor modifications for potential inclusion in this pilot project.
Larger scale modifications, such as ramps or showers, will be assessed and documented. If the participant wishes to pursue these modifications, a referral will be made to the consumer’s care coordinator for a Medicaid E-mod waiver application.
Assistive technology is are interactive devices, sometimes connected to the internet, that address an individual’s needs in maintaining or improving their ability to perform activities of daily living, control or access their environment and communicate with the community. Assistive technology devices may include: alarms, sensors, cameras, motion detectors, GPS trackers, home security systems, door and window alarms, and two way communication systems.
Both the Home Accessibility and Assistive Technology assessments will be conducted in the participant’s home. Once the assessments are completed, the participant will receive a written report of recommended solutions specific to their individual needs. The reports will offer suggestions of services, modifications, independent living, and assistive technology products that, if obtained, would allow the participant to live at home longer, safer, and more independently.
Medication dispensing devices automatically dispense medications and also remind individuals when to take their medicine. These devices have the potential to help individuals with complex drug regimens take their medications on time and in the correct doses.
Home Telehealth Monitoring is a broad category of devices that work with individuals who have a history of hospitalizations for chronic conditions. The devices are able to send daily vital signs to caregivers, providers, and medical professionals from the participant’s home.
