As a volunteer on a UH project, you are asked to complete and sign a couple of forms, described and linked below. These forms should be completed and submitted before you work on the project and may be required to be reimbursed for travel expenses.
The forms are in pdf format, but they are not fillable. You may drop off a hard copy to our department secretary, Lisa Kam, in Sherman 101. If you prefer to submit an electronic copy, please submit them via UH Filedrop to nremsec@hawaii.edu to protect your confidential information.
This document includes the University of Hawaii policy on use of volunteers to assist with UH projects. It includes the rights and responsibilities you have as a volunteer, including the ability to be reimbursed tax-free for eligible travel expenses. At the end of the document is Attachment 1, an application form in pdf format you need to complete, sign, and submit to your volunteer supervisor for their signature.
This form is also required of volunteers to ensure you agree to the assumptions of risk as part of your work and provide UH consent to provide medical treatment along with your medical insurance information.
If you will be incurring travel or other reimbursable expenses as part of your volunteer work, you will need to fill out this form to be reimbursed. This form asks for citizenship and federal tax status. If you are not a US citizen or permanent resident, you will be required to indicate your visa status and recent history and submit verifying documentation.
Note that volunteers cannot receive a travel advance and do not need to submit a travel request. Reimbursement for travel can be requested through our travel completion form.
