Plan to Submit by August 1
All students who will be enrolling after Summer 2023 will need to submit their Health Clearance through Med+Proctor. This is no longer through the University Health Services Manoa.
July 31 - Remove Health holds. Health holds are automatically placed on the MyUH accounts of newly admitted, returning, or transfer students to the University of Hawaiʻi. To remove the holds, you must submit the Health Clearance Form to Med+Proctor using these instructions. If you are unable to locate your records, you may need to make an appointment with a doctor to update your immunizations. Plan to submit your forms before August 1 to allow Med+Proctor time to process your submission before the first day of the semester.
Those enrolled in Fully Online Programs are exempt from submitting Health Clearances. However, if you wish to participate in on-campus employment and/or activities, you will need to submit this.

Tips for Recent Graduates:
If you attended any UH Campus prior to July 2020, you are only required to have Measles, Mumps, Rubella (MMR) and Tuberculosis (TB) Clearance. Please log on to your STAR account to see if you've met this requirement.

If you have recently submitted this form, please monitor your MyUH account until the health clearance holds (ex., MMR Clearance Hold, TB Clearance Hold, Health Clearance Hold) are removed. Log in to MyUH and click on "View Holds on My Record" or use the search bar at the top.
