Letters of Residence from Landlord in Lieu of Lease Affidavit 7:60-AP-2-E-1  English

Required if a parent or guardian seeking to enroll a child needs evidence of residency when a signed lease is unavailable.  This includes an expired lease that has automatically become a month to month tenancy.  Other documents will also be required to establish residency.

Letters of Residence Affidavit 7:60-AP-2-E-2 English

Required if a parent or guardian seeking to enroll a child is living with a District Resident, such as friends or relatives, and they cannot provide sufficient proof of residency in their own name.

• To be completed by the parent or legal guardian and the District Resident

• District Resident is required to provide a minimum of two (2) verification documents required to establish residency

• Parent or legal guardian will be required to provide a minimum of one (1) verification document from Category B (see Residency Documents for Address Changes below)

• The School District reserves the right to evaluate the evidence presented, and merely presenting the items listed in this procedure does not guarantee admission

Original Birth Certificate, Certified Copy, or Birth Certificate Visa when applicable

A certified Birth Certificate is issued from the county office, in the county where the child was born. An official Birth Certificate will be marked with a county stamp or crimped seal.  A certificate from the hospital (including a certificate with footprints) will NOT be accepted.

Copies will not be accepted.  Our staff will make copies for school records

7:30-E Exhibit - In-District Transfer Request

The Harlem School District provides parents with an opportunity for their children to attend an alternate school other than their neighborhood school under certain circumstances, providing there is sufficient space in another Harlem elementary school.

Conditions of Transfer

Transfers will only be considered if the child being transferred has a sibling currently attending the

requested/receiving school.

• Transfers considered will only be approved IF:

• Building and class enrollments can be maintained at an acceptable level.

• Applicants follow proper procedures, student registration is complete, and the request is made at District Wide Registration, which this year is online BEFORE June 2 or by the child’s first attendance day if the child is transferring into the district during the school year.

• Parents provide all transportation.

• The conduct, attendance, and progress of the student must be acceptable at the receiving school or the transfer may be revoked at any time.

Steps for In-District Transfer Requests

Steps for new requests:

1. Parent submits In-District Transfer request form by May 31st (returning students) or June 30th (new students).

2. Parent registers with home school (neighborhood school) during the registration window (May for returning students and June for new students).

3. All requests are sent to the assistant superintendent’s office for consideration.

4. Elementary principals meet with the assistant superintendent to review enrollment numbers and determine which In-District Transfers can be approved.

5. In most cases, parents will be notified of the decision within a week of the first day of school. The final approval date will depend on enrollments.

6. Renewal procedure:

Once the student has transferred to another school, they do not have to resubmit a request for transfer on an annual basis.

Students may continue at the receiving school until they complete sixth grade as long as their conduct, attendance and progress are judged by the principal to be at an acceptable level. The principal will notify the parent of his/her decision to discontinue the allowed transfer.

The administration reserves the right to terminate a transfer at any time. If students or parents fail to comply with conditions of transfer, the student will be returned to his/her neighborhood school.

Age

To be eligible for admission, a child must be five years old on or before September 1 of that school term. A child entering first grade must be six years of age on or before September 1 of that school term. Based upon an assessment of a child’s readiness to attend school, the District may permit him or her to attend school before these dates. A child will also be allowed to attend first grade based upon an assessment of his or her readiness if he or she attended a non-public preschool, continued his or her education at that school through kindergarten, was taught in kindergarten by an appropriately licensed teacher, and will be six years old on or before December 31. A child with exceptional needs who qualifies for special education services is eligible for admission at three years of age. Early entrance to kindergarten or first grade may also be available through Board policy 6:135, Accelerated Placement Program.
