Applying to College Board for test accommodations for the PSAT, SAT and AP testing
Consent Form for College Board Accommodations Request: Please complete this form and submit to your child’s Teacher of Record in 9th grade. The Teacher of Record will submit the necessary documentation to the College Board for their determination of accommodations.
Applying to ACT for test accommodations
Please email your child's guidance counselor, after you have completed the entire ACT registration form to make a request for testing accommodations.

Applying for test accommodations at the collegiate level:
Students who have been utilizing test accommodations during high school are encouraged to apply for accommodations at their college of choice prior to their freshman year.
