Transition services are a coordinated set of activities. These services are focused on improving the academic and functional achievement of all children with a disability. They help a child move from school to post-school activities. Transition services include:
instruction
related services
community experiences
the development of employment and other post-school adult living objectives, and
the acquisition of daily living skills and
functional vocational evaluation.

The Academy Model offers ALL students:
Career awareness, exploration, and immersion opportunities (Work-based learning)
Goal setting and planning
Workplace readiness skills building
10-year college, career, and life ready plan (also known as the Academic Career Plan, ACP)
Additional transition support is available for students whose Individualized Education Program (IEP) team has determined a need beyond what the academy model provides. This support can be provided on an intermittent basis or on a more intensive, ongoing basis. Identifying transition services begins with a transition assessment and data-driven conversations with:
the student
the parent/guardian
school staff
Discussion points are student-centered based on the student's goals and assessments (ex., education, career, and adult living). Services and support are determined and finalized by the IEP team based on student need and reflective of a student's interests, skills, and passion.
Employment and Transition Services may include:
instruction on a variety of transition topics
vocation assessments and exploration
work-based learning, including job coach opportunities
school-based entrepreneurship
job site development and job placement for students within the community
volunteer and leadership opportunities with business partners
specialized training for staff and families on transition topics and the transition IEP process
referring students to HCS classes and services
connecting students with adult agencies
connecting to tutoring, mentoring, and extended learning opportunities

300.43 Transition services.
(a) Transition services means a coordinated set of activities for a child with a disability that—
(1) Is designed to be within a results-oriented process, that is focused on improving the academic and functional achievement of the child with a disability to facilitate the child’s movement from school to post-school activities, including postsecondary education, vocational education, integrated employment (including supported employment), continuing and adult education, adult services, independent living, or community participation;
(2) Is based on the individual child’s needs, taking into account the child’s strengths, preferences, and interests; and includes—
(i) Instruction;
(ii) Related services;
(iii) Community experiences;
(iv) The development of employment and other post-school adult living objectives; and
(v) If appropriate, acquisition of daily living skills and provision of a functional vocational evaluation.
(b) Transition services for children with disabilities may be special education, if provided as specially designed instruction, or a related service, if required to assist a child with a disability to benefit from special education.
