Middle School students are required to provide a vaccine record for the following immunizations to enter Middle School. These booster vaccines are given at 11 years old by your health care provider.
Tdap (Tetanus, diphtheria, acellular pertussis):

A message to incoming 6th grade parents
Please note there will be no grace period for the required 6th grade vaccinations. If your child's annual well visit is after school starts, you will need to make a doctor's appointment specifically for these immunizations before school starts in September. If your child turns 11 over the summer, schedule an appointment for the immunizations as soon as possible after their birthday.
