Use this form to self-disclose your disability. This is for students who have been accepted to Guilford (note: you will need to be logged into Guardian). Prospective students should email accessibility@guilford.edu
Option A: Gather any existing documentation that supports your request. This could include medical records or test results, a recent IEP/504, psycho-educational evaluation, or a letter from your doctor, therapist, or caseworker.
Option B: You can download and send this form to your provider (must be a qualified professional familiar with your diagnosis and current functioning).
Additional forms or documentation may be required for some accommodations (like emotional support animals)
After completing steps 1 and 2, meet with an SDAS team member to discuss your needs. Appointments may be virtual or in-person.
Students should contact SDAS as soon as they have been accepted to Guilford AND are aware that they need accommodation. Referrals and support happen all year long!
Academic Accommodations
There is no fixed deadline for requesting academic accommodations. Still, timing can affect your experience. For example, if you wait until after the semester begins, you may be unable to make certain schedule changes or could experience a delay accessing your accommodations.
Housing Accommodations
Students should request housing accommodations when they fill out the next semester's housing application. There is no fixed deadline for requesting academic accommodations, but timing can affect your experience. For example, there may be limited selection available, or an impact on your billing.
Emotional Support Animals (ESAs)
ESAs are not approved after the census date for the requested semester. Exceptions will be made for emergencies and newly emerging needs. You can find the census date on our academic calendar!

Documentation will look different based on the student and their unique situation. SDAS staff will guide you along the way. We also understand that access needs are not always fully reflected in documentation alone. Student interviews, prior accommodation history, and information gathered through the interactive process may be used to supplement - and in some cases, may serve in place of - outside documentation when appropriate.
