How to Request Accommodations

SDAS uses an interactive process to identify accommodation and support needs.  This means the student is involved from the very beginning and has a leadership role. 

Step 1: Request Accommodations

Use this form to request accommodations. Current students must be logged into Guardian to open this form. 

Step 2: Provide documentation

SDAS considers information from a variety of sources.

Option A: You can download and send this form to your provider (a qualified professional who is familiar with your diagnosis and current functioning). 

Option B: History of your disability and evidence of your current functioning. May include medical records or test results, a recent IEP/504, a psychoeducational evaluation, or a letter from your doctor, therapist, or caseworker. 

• • Additional forms or documentation are required for some accommodations (such as housing and ESAs/emotional support animals, which require Option A or a letter that meets the same criteria)

Step 3: Complete an intake appointment

After completing steps 1 and 2, you will meet with an SDAS team member to discuss your needs. Appointments may be virtual or in-person.

Guidance on Timing and Deadlines

Students should contact SDAS as soon as they have been accepted to Guilford AND are aware that they need accommodation. Referrals and support happen all year long!

Academic Accommodations
There is no fixed deadline for requesting academic accommodations. Still, timing can affect your experience. For example, if you wait until after the semester begins, you may be unable to make certain schedule changes or could experience a delay accessing your accommodations.

Housing  Accommodations
Students should request housing accommodations when they fill out the next semester's housing application. While there is no fixed deadline for requesting accommodations, timing can affect your experience. For example, there may be limited selection available, or an impact on your billing.

Emotional Support Animals (ESAs)
New Students (FYs and Transfer): ESAs are not approved after the census date for the requested semester. Exceptions will be made for emergencies and newly emerging needs. You can find the census date on our academic calendar!

The Role of Documentation

Documentation will look different based on the student and their unique situation. SDAS staff will guide you along the way. We also understand that access needs are not always fully reflected in documentation alone. Student interviews, prior accommodation history, and information gathered through the interactive process may be used to supplement - and in some cases, may serve in place of - outside documentation when appropriate.
