Marching Band Only Forms

ALL ATHLETES:

• Go to santana.guhsd.net

• Click on the Athletic Clearance under Quick Links.

• New Users:  Register for a new account and then proceed to login.

• Returning Users: Login

• Complete the Online forms. Please make sure to select school year 2024-2025, Santana, Sport

• Print and sign the Confirmation/Consent form for each sport. Parent and Student sign.

• Bring the signed form and a current physical to the Finance Office.

• This process must be done for each sport. However, once a current physical is on file, additional copies are not necessary to provide with consent form.  

• If you are experiencing trouble accessing the website and you are using Google Chrome – switch to Internet Explorer or Firefox.

INSURANCE:

• All athletes MUST have current health insurance to try out and participate.  Please do not leave Insurance Company and policy number blank on the athletic clearance process.

• If you need to purchase insurance to participate in a sport, plans are available to purchase at:    https://www.k12specialmarkets.com/Enroll_1.aspx or find the link on the Sultan website under Student Insurance.

PHYSICALS:     

• Physical Form is available for download on the Athletic Clearance webpage or in Finance.

• Parent completes top section of form. Signature is required.

• Physicals are required annually and must be valid throughout the entire season of your sport.

• Please have Doctor and Medical Provider stamp and sign the completed physical form.

• Physicals are good for one year from the date of the exam – not the date the physician completed the form.
