CLEARANCE PROCESS - 

HOW TO CLEAR SO YOU CAN TRY OUT!

  ⇨ All athletes must 

(Step 1) complete the on-line clearance process (print and sign it after completing the online process) 

AND 

(Step 2) bring the signed online clearance signature page WITH a CURRENT physical (USA MD or DO signed & stamped) to the Student Support Center. 

⇨ You will then give the student a “clearance verification form”.

 (This will be a stamped sheet of paper with a circular “cleared” stamp. It will also be initialed by an MVHS employee.)

⇨ This stamped/initialed “clearance verification form” is what is REQUIRED to try-out. This verification form is given to the coach on the day of tryouts.

No stamped clearance verification form = no try-out!
