CLEARANCE PROCESS -
HOW TO CLEAR SO YOU CAN TRY OUT!
ALL athletes must:
⇨ Complete the on-line clearance process. Print and sign the verification page at the end of the on-line process.
⇨ Bring the signed verification page WITH a CURRENT physical (USA MD or DO signed & stamped) to the Student Support Center prior to tryouts.
⇨ You will then receive a "Monte Vista COMPLETED Clearance" form. This form is what is REQUIRED to try-out. This verification form is given to the coach on the day of tryouts.
No stamped clearance verification form = no try-out!
YOU WILL KNOW YOU ARE CLEARED WHEN YOU RECEIVE A PIECE OF PAPER FROM THE STUDENT SUPPORT OFFICE THAT HAS THIS COMPLETED STAMP!
