f = first name initial
l = last name initial
mm = two digit birthday month (e.g. January would be 01)
dd = two digit day (e.g. the 6th would be 06)
yy = last two digits of year (e.g. 2005 would be 05)
Example: If my name is John Brown and my birthday is January 7, 2004, my password would be jb010704
