If your work involves human participants, vertebrate animals, potentially hazardous biological agents, and hazardous substances and devices, you must have a supervisor. Your supervisor must fill form 2 BEFORE experimentation ends.

If your work involves human participants, vertebrate animals, potentially hazardous biological agents, and hazardous substances and devices, you must have a supervisor. You AND your supervisor must fill form 3 BEFORE experimentation starts.

If you are working with human participants not at a regulated research institution, you must complete the top of form 4 BEFORE experimentation starts. You must see Ms. Berry for project approval.
