All forms need to be printed and filled out by a guardian. 

The physical must be filled out and dated by a physician.

Forms should be returned to the correct coach or athletic director.

Students may not participate in any athletic activities until they have turned in all paperwork and physical. 

Player insurance is only paid after a student makes an athletic team.
