This form is intended to allow parents and students to request a Retake or Redo on any MAJOR assignments.
Please fill in all questions on the form. Once received, your teacher will contact the student regarding the required re-learning opportunity and a redo or retake date.
The student must:
Submit the teachers Retake/Redo Request Form within five school days of the grade being posted.
Have submitted all minor assignments associated with the instructional unit.
Complete teacher-directed remediation or relearning before reassessment.
Coordinate the reassessment with the teacher during an agreed-upon intervention time.
Student can only retake Major assessments once.
The highest score will be recorded in the gradebook.
