Greenville County Schools Student Science Safety Contract

This is to certify that I, _____________________________________, have been instructed in, and understand, the following safety components of science class.

*Please initial on the right side of the paper to show that you have received information from your teacher and understand the rules, location of safety equipment, procedures, and appropriate lab behaviors.

Safety Rules & Behaviors

Initial

1. Use lab only when directed by the teacher.

2. Never work with chemicals without checking labels carefully, and only when directed by the teacher.

3. Place broken glass and disposables in appropriate designated containers.

4. Report any accident, incident, or unsafe situation to the teacher. Monitor safety precautions for fellow students.

5. Never taste, touch, mix, or inhale substances except as directed by the teacher.

6. Confine long hair and loose clothing whenever working with flame or chemicals.

7. Close-toed shoes should be worn on lab days. Open-toed shoes are not to be worn during lab work where there is a potential for harmful objects or chemicals to fall on the foot (feet).

8. Do not eat, drink, or chew gum in lab areas.

9. Wash your hands before leaving the lab.

10. Wear safety goggles for labs when chemicals and heat are used as instructed by your teacher. If contacts are worn, avoid contact with corrosive vapors.

11. Do not horse play, throw things, or joke around in the lab. Always demonstrate proper lab technique.

12. I understand that in lab I am expected to stay at my assigned station / table / area, unless otherwise directed by the teacher.

13. At the completion of the lab, I understand that I should clean my area, dispose of all chemicals properly, return materials, and start working on my next assignment.

14. I understand that I should follow additional safety rules that may apply in each lab as directed by my teacher or the lab directions.

To the Parent/Guardian: Your young adult will be working in the laboratory during this course. In order to ensure his or her personal safety, it is important that the above rules are followed. Failure to do so may result in your student being removed from lab activities. Please review these important rules with your student as I have in class.
