📝 How to Make a Referral

Referrals to the RBHS (Rehabilitative Behavioral Health Services) program can be made by school counselors, teachers, or administrators.

 Complete an Online Referral Form:

📅 Intake Process

A parent or guardian must attend the first appointment and sign required consent forms.

Appointments are held at Blue Middle School 

Plan for the intake to last 60 to 90 minutes.

Bring a Medicaid card if your child has one.

If your child does not have Medicaid, services are still available at no cost.
