Mrs. Smith's Fourth Grade

We need to know why students are absent. Without any communication, those days will be recorded as unexcused. You may submit the reason for the student's absence by ONE of the following methods:

1.Electronically through our Absentee Google Form

2. A hard copy, a scan, a email or a fax of a Doctor's note

3. An email or a handwritten note from the student's legal guardian. Please include the following details in your note: Student’s legal first & last name, Teacher/Grade, Date(s) missed, Reason for the absence, Parent or guardian’s signature

Parent MUST fill out on the DAY OF a transportation change by 1:30 pm. Any changes submitted after 1:30 pm may not be guaranteed.

*IF YOU ARE SENDING SOMEONE ELSE TO PICK YOUR CHILD UP PRES REQUIRES A CAR TAG OR A DRIVERS LICENSE FOR VERIFICATION*

*ALL K5 & 1ST GRADE STUDENTS HAVE TO HAVE A CAR TAG AT THE BUS STOP IN ORDER TO BE RELEASED FROM THE BUS
