INTERSCHOLASTIC ATHLETIC PROGRAM

You have been given a wonderful opportunity to represent Woodlands Middle/High School on the playing field which first requires you to be successful in the classroom. The foundation for your academic and athletic success will require you to be focused, determined, dedicated and disciplined. It will demand you to successfully balance your effort and manage your time between your academic and athletic responsibilities.

The Greenburgh CSD offers a comprehensive athletic program that is committed to the development of physical fitness, personal growth, safety and achievement of excellence for our students. Please take advantage of the unlimited diverse opportunities to grow in all aspects of life and remember that representing Woodlands Middle/High School is a privilege. The athletic department takes great pride in the accomplishments of our student‐athletes and expects our student-athletes to be leaders on their team, in our schools, and in our community.

Any academically eligible student in grades 7-12 may try out for an interscholastic team. There are three sports seasons throughout the year (fall, winter and spring), and a student may participate in one sport per season. The below requirements must be met before participating. (The Health Appraisal and Medication forms along with concussion information can be found under the “Athletics” tab on the Greenburgh website or under the “Links” tab in FamilyID.)

HEALTH APPRAISAL FORM (CURRENT PHYSICAL)

A physical exam is required for medical clearance and must have been completed within 12 months prior to the start of any sport season. It is preferred that the exam be performed by your healthcare provider; however, if you encounter any problems, contact the Health Office. You may either upload the completed physical through FamilyID or submit a hard copy to the Health Office.

HEALTH/MEDICAL HISTORY

This online form is mandatory and must be completed and e-signed by the parent/guardian no more than 30 days prior to the start of each athletic season. The Health/Medical History is valid for one season; the form must be updated for any student who participates in multiple sport seasons.

EMERGENCY CONTACT AND CONSENT FORM

These online forms are mandatory and must be completed and e-signed by the parent/guardian. Any informational changes can be updated as needed.

MEDICATION ADMINISTRATION FORMS

The online form(s) must be completed by your physician and submitted to the Health Office if your child requires medication (inhaler, EpiPen, etc.) during practice or competition. All medication must be in the original packaging and given to the coach by the parent/guardian.

CONCUSSION MANAGEMENT AND AWARENESS INFORMATION

Required reading for the student-athlete and parent/guardian.
