Instructions for completing the Community Eligibility Provision (CEP) Form in IC

Step-by-Step Instructions

1. Log In to Infinite Campus [1]

• Open your district's Infinite Campus Campus Parent portal or use the Campus Parent mobile app.

• If you do not have an account, click the link on the login screen or contact your local school district office for your portal activation key. [1]

2. Locate Meal Benefits

• From a desktop: Go to the left-hand menu and click More, then select Meal Benefits.

• From the mobile app: Tap the three horizontal lines in the top left corner, select More, then tap Meal Benefits. [ 1]

3. Complete the Application Steps
Follow the on-screen prompts, which will guide you through the following sections: [1, 2]

• Review Household Members: Ensure all children, students, and other adults living in your household are listed. Add any missing family members.

• SNAP/KTAP Benefits: If anyone in your household receives government assistance like SNAP, enter the 10-digit case number.

• Income: Enter the gross income and pay frequency for all working adults and children in the household (or check the box if they have zero income). 

• Authorization & Demographics: Select whether your information should be shared with other programs (like Medicaid) to qualify for additional benefits. Race/ethnicity questions are optional.
