Weekly Report

Each site will be required to complete a FFVP Meal Service Report for each FFVP Service day. Please submit the FFVP Meal Service Reports by Friday each week. Please be sure to have the reports for the last week of every month submitted by the due date especially before the holiday breaks.

Additional Information:

1. The Weekly Reporting Form will automatically collect the email address of the person completing the form.

2. The Weekly Reporting Form will ask for a date. Please put the serving date not the day you are submitting the report.
