Please be aware: 

Prior to receiving any Body Art Services and/or procedure(s) from GRADE A INK TATTOO AND PIERCING STUDIOS, it is essential to carefully read, complete, and submit our Waiver, Release, Assumption of Risk, Indemnity, and Consent Agreement. We kindly request that you refrain from signing this agreement unless you have thoroughly reviewed its contents in their entirety, and fully accept, and agree to all terms outlined within. While you may review the agreement well ahead of your appointment, it must be signed, dated, and submitted at the time of your appointment. Services will not be administered unless the Agreement is signed, dated, and submitted. No exceptions will be made.

(Please Note: For each new service/procedure, we require a newly signed, dated, and submitted Agreement before service/procedure is provided.)

If Parents or Legal Guardians consent for their minor child to receive service(s) and/or procedure(s) allowable by the State Laws (please refer to the State Laws listed on this Agreement), both parent and minor must read, agree, and sign this Agreement, and the "Schedule B" form attached. **Parents/Legal Guardians must remain with the minor, the entire time the minor is receiving services and/or procedures. No exceptions!**

(This is to ensure your safety and well-being as our valued client!) 

*****The Artist performing service(s) and/or procedure(s) shall witness the client's signing and dating this Agreement by also signing and dating it themselves after the client, below the client's section. 

Alternatively, the Concierge checking clients in may witness the client's signing and dating of this Agreement (and attachments if applicable).
