The following must be completed and on file with the athletic training staff prior to a student participating in an athletic period workout, practice or athletic competition:
A current medical history & physical completed by a medical provider (MD, DO, PA-C, NP)
The following online forms must be completed via Rank One:

This form must be completed by all students in grades 7-12 who are participating in an extracurricular activity for GISD to include athletics, band, cheer, dance, FFA, UIL academics, theater, etc.

FREQUENTLY ASKED QUESTIONS
1. Does my 6th grader need a physical for pre-athletics?
No. Only incoming 7th-12th, athletics, marching band, cheer, dance, or anyone new to athletics/district. GISD Athletics, Band, Cheer, and Dance requires an annual physical
2. I got my child's physical done at their pediatrician's office. Who can I drop it off to?
You can now upload your child's physical via Rank One or you can drop it off at the high school athletic office at the fieldhouse or you can email it to jeremy.philips@gonzalesisd.net with your child's name in the subject line. Please ensure both the medical history (front page) & physical exam (back page) are submitted when turning them in. Incomplete documents will not be accepted
3. Why is my child showing that their physical is missing in Rank One but I turned it into the athletic office/gmail?
We input physicals as we receive them. Understand that each physical has to be input individually by the athletic training staff and approved if it is upload by you into Rank One. If your child's physical is still not showing up in Rank One by 1 week after it was turned in please email jeremy.philips@gonzalesisd.net
4. I am trying to fill out my child's Rank One form but it is telling me that they do not exist.
If you are new to the district then we will need to enter you into the system manually. Please email us at jeremy.philips@gonzalesisd.net and we can get some information from you to get your child in our system. They must have a Gonzales ISD student ID in order for us to get them into the system.
5. Does my child's physical have to be on the UIL form?
Yes. This form is required for all schools (public) that participate in programs under the University Interscholastic League. No other form will be accepted
6. I need to talk to one of the athletic trainers. What is the best way to contact them?
The best way to contact us is by emailing us at jeremy.philips@gonzalesisd.net. We are not at the office during the summer everyday. However, the training staff will check this email daily and respond as quickly as possible.
**The athletic training staff is NOT responsible for any physicals that are not turned into the athletic office or athletic training gmail.**
**IT IS STRONGLY ADVISED TO UPLOAD THE MEDICAL HISTORY & PHYSICAL INTO RANK ONE OR TURN IN THE PHYSICAL PAPERWORK TO A MEMBER OF THE ATHLETIC TRAINING STAFF AT THE GISD FIELDHOUSE.**
