• Check in and support students with transitions

• Participate in transition meetings with student planning teams

• Develop a grade 6 learner profile with students to support self-advocacy

• Raise awareness of student's strengths and challenges

• Implement and support effective compensatory strategies

• Practice self -advocacy

• Encourage sports, clubs & extra curricular activities
