REQUEST FOR PAYMENT FORM – To be completed whenever a payment is required by a team or coach.

*** Deposit Envelopes are in the main office for teacher liaisons and coaches. ***

J FORM -To be filled out by all adult drivers who transport student athletes and turned into the Athletic Director or the school administration office.

Form A Trip policy Day Trips – To be filled out by coaches/advisors for teams who will be playing in an away tournament or exhibition game( outside of our metro league) but will not be staying overnight. Please turn into Athletic Director.

Form B Overnight Trips- To be filled out by coaches/advisors for teams who will be traveling overnight, out of province or out of country and turned into the Athletic Director.

REVISED Field Trip Consent Form – March 2015 (4) – This is our permission form for trips( both day trips and overnight trips) that must be signed by a parent/Guardian.

C FORM- To be filled out by coaches/advisors for classes/teams who will be canoeing, skiing, hiking, rappelling, orienteering and turned into the Athletic Director.
