Per Glen Ridge school policy, all incoming new to the district elementary students and all Third and Seventh-Grade students are required to have a current physical on file in the nurse's office. The universal health care record must be completed and signed by the physician.
If your child has allergies that require emergency medications, please have the student's physician complete the allergy action plan and medication authorization. Two Epi-pens, Benadryl, and the completed paperwork can be dropped off at the school in a ziplock bag labeled with your child’s name and grade. This can be brought into school late in the summer, before the first day of school.
If your child has Asthma, please have the student’s physician complete the asthma treatment plan. The rescue inhaler and spacer will be kept in the Nurse’s Office.
It is a New Jersey State requirement that all medication orders must be renewed annually. Please notify me if your child requires medication at school. All medications must be accompanied by a medication form and hand-delivered by an adult in the original container from the pharmacy with the child’s name and instructions on the label.

Any injuries requiring crutches, braces, or Physical Education exemptions require a note from the student’s physician. The form can be found on the District Nurses’ Website under return to school or physical education form. The medical professional may also supply their own.
