Administrative forms

Sports Report Form – Each athletic team has one or more Team Representatives who perform many essential services both to the GBN Booster Club and the teams they represent. All Team Reps are asked to complete a Sports Report Form and present it at our monthly meeting while your sport is in season. If you cannot attend a meeting, please email your form to teamrep@gbnbooster.org c/o our Secretary. Even if you are attending a meeting to give your report in person, please also email the form.

Reimbursement Form – For any Board Member purchasing any item for the GBN Booster Club, please complete a Request for Reimbursement Form, attach your receipt, and email both to treasurer@gbnbooster.org for reimbursement. Please remember to request and use the Tax Exempt Letter when making any purchase—you will NOT be reimbursed for tax.

Deposit Form – Please complete the Deposit Form if you have cash and/or checks that need to be deposited, and email your form to treasurer@gbnbooster.org.
