Verifying Dependent Eligibility

If you have added a new dependent to your coverage, you will receive a mailing from the Dependent Verification Center in Lincolnshire, IL in early January. The letter will specify the documentation required to confirm your dependent’s eligibility and the deadline to respond. To avoid termination of your dependent’s coverage, you must respond by the deadline indicated in the letter.

Dependents’ Social Security Numbers Are Required

Social Security Numbers (SSNs) for dependents must be entered in Workday. Federal law requires the Company to collect the Social Security Numbers (SSNs) of anyone covered under our plans for mandatory reporting to the IRS. Please review your dependents’ SSNs in Workday and enter any that are missing. From the Workday home page Menu, click Benefits > then Dependents > and edit each dependent. If you make changes, click Save; otherwise, click Cancel.
