The mission of GG's Wellness Centers, Inc. is to provide relevant resources such as job training, mentorship, entrepreneurial skills, and housing to the population that has suffered due to economic challenges, Poor decision making, and unfortunate circumstances by bridging the language, economic, skill, mental, and social gaps that are seen as barriers or obstacles to their paths to success.

"CONTINUUM OF CARE":
STEP 1 : Identification and Assessment Identify individuals who are at high risk of recidivism and homelessness. This can be done through a screening process that involves interviews, assessments, and evaluations. The assessment should focus on the individual's needs, strengths, and weaknesses.
STEP 2: Case Management
Once identified, each individual should be assigned a case manager who will work with them to develop a comprehensive case plan. This plan will address their specific needs, such as mental health, addiction, education, employment, and housing.
STEP 3: Transitional Housing
Transitional housing is a vital component of the continuum of care plan.
