Physicals & Forms

The 2024-2025 Physical Forms are on the Georgetown ISD athletics website.

A student will NOT BE CLEARED for participation in tryouts, in-season or off-season practices UNTIL ALL OF THE REQUIRED ITEMS BELOW HAVE BEEN SUBMITTED. 

REQUIRED ITEMS FOR PARTICIPATION 

1. Physical/Medical History Form (Submit in Person)

2. Catastrophic Insurance (Submit in Person)

3. Rankone Electronic Form (Submit Electronically)

4. Georgetown ISD forms (Submit Electronically)

Parents please visit Georgetown ISD Athletics for full details on each of the required items to be submitted and to submit the online portion of the paperwork. The online portion can be submitted anytime, so we highly encourage parents to complete that portion before the school year is over.
