GWHS LIBRARY CARD APPLICATION FORM

This form needs to be filled out in order to receive a GWHS Library Card. A Library Card is needed to check out library books and other materials. The first library card is provided at NO COST. Lost Library Cards will be charged a fee of $3.00 in order to be replaced.

TECHNOLOGY FORMS and MEDIA RELEASE FORM

ALL students must turn in a Education Technology Use Form and an Appropriate Usage Form in order to use the library equipment and the school network. Please turn in these forms to the school librarian in order to ensure accessibility to our tech resources.

BP 379 - Education technology USE POLICY

The primary purpose of the GDOE communication network is to support and enhance teaching and learning to prepare all students for success in lifelong learning. BP 379 provides information on the rules and regulations regarding the usage of the GDOE network and its provided technology.

If you would like to reserve the library for your class, please complete the Library Reservation Form.

If you would like to reserve equipment / tech supplies for your class, please complete the Equipment Reservation Form.

Please provide at least 1 day to confirm your reservation. Once reviewed, you should receive an email confirming your reservation. Thank you!

LIBRARY BOOK RECOMMENDATION FORM

GWHS students, faculty, staff, and community patrons are invited to recommend materials (books, DVDs, journals, etc.) to be considered for purchase by the GWHS Library in order to improve its collection. The decision to buy or not buy any specific item in your request is based on the Library’s collection development policy, the item’s usefulness to the collection, and budgetary constraints.
