GDOE schools require compliance with the current Department of Public Health and Social Services (DPHSS) recommended immunization schedules for school entry and attendance.
Medical and religious exemptions must be approved by DPHSS. Contact the DPHSS Immunization Program at 735-7143 or 735-7148 for more information. Religious exemptions must be updated each school year.
TB evaluation must be done within 12 months of registration.
Tuberculin Skin Test (TST) date of administration and date of reading must be clearly documented.
TB clearance form issued by DPHSS is required for attendance when student has a positive result.
TB clearance form can be obtained from DPHSS TB Program at the Northern Region Community Health Center (NRCHC) located in Dededo. Provide the LTBI form from your child's Health Care Provider and your child's Chest X-ray result to the program. TB program office hours are limited. Contact the School Nurse for more information.
Physical exam must be done within 12 months of registration.
Annual physical exams are encouraged. Please send updated documents to the School Nurse.
Student Emergency and Health Forms are given at the beginning of each school year and upon registration.
Parents/guardians should submit the completed form to the School Nurse as soon as possible.

Follow-up immunizations to complete the series as required by the current Immunization Advisory provided by the Department of Public Health and Social Services (DPHSS)
Updated tuberculosis (TB) evaluation upon entering 6th and 9th grade
Updated Physical Examination form upon entering 6th and 9th grade
Students who are not in compliance with these requirements may be excluded from school until valid documents are submitted. Contact the School Nurse for questions or concerns.
