Absence Requests

This form is to be filled out by a PARENT/GUARDIAN for any band rehearsal/performance absences. This form should be filled out in a timely manner and submitted well in advance of the absence to ensure the absence is approved. 

Families are strongly encouraged to review the band program handbook for policies regarding excused and unexcused absences, especially those regarding band performances.
