required forms
There are many forms that need to be completed to satisfy requirements from both Trees for Tomorrow as well as the Green Bay Public School District. Please have the following information available before completing the forms: 3 emergency contact names and phone numbers, physician information (including date of last visit), health history, insurance information (including hospital preference), medication names and dosages. All forms must be completed in order for a student to attend the trip.

- student contract
- medical release form
- GBAPS paperwork
- overnight field trip permission
- personal health history and emergency info
- field trip acknowledgement and right to search
- authorization of consent for treatment
- medical authorization form for EACH prescription or OTC medicine
