It is VERY important that you complete a medical authorization form for EACH medication your child needs or even MIGHT need.

Over the counter medicines only need your signature (as long as you don't want your child to exceed the recommended instructions).

Prescriptions require your doctor's signature. The district nurse is happy to facilitate this for you as long as you get the forms in by the deadline.

Place all medications in a plastic tupperware container with your child's name written clearly.

In addition to prescription medications, PLEASE SEND ANYTHING YOUR CHILD MAY POSSIBLY NEED. Some common issues that have occurred in the past include the following:

  • anxiety or panic attacks
  • headaches
  • toothaches
  • skinned elbows or knees

I am also requesting that ALL parents sign a form so I can give your child pain reliever if needed. It is a horrible feeling to see your child in pain and not be able to give him or her pain reliever because I do not have the medical form.

In order for me to give your child over the counter medication, I need the following two items:

1. a medical authorization form signed by you

2. the medication in the original packaging
