(นสพ.ไม่ต้องยื่น) อาจารย์ผู้สอนใช้ใบประเมินจากภาควิชา ฯ ประเมิน นสพ.ในชั่วโมงเรียน
3.แบบประเมิน OPD (ตัวอย่าง)
นสพ.ยื่นให้อาจารย์ผู้คุมประเมินทุกครั้งที่ออก opd (1 ใบประเมินหากอาจารย์ท่านเดียวกันคุม นสพ.หลายคน ใช้ใบเดียวกันได้เลย)
