Cancellations and Late Testing

This form is only available during February. Be sure to complete it before the deadline.

 Provide Required Information

• Student Name: Enter the student’s full name.

• Student Number: Enter the student’s school identification number.

• Email for Correspondence: Enter an adult’s email address. This is required because all cancellations or changes must be confirmed in writing via email.

Select the AP Test(s):
Choose the test(s) for which this request applies from the provided list. Select "Other" and specify the test if it's not listed.

Specify the Request Type
You can select the appropriate option to indicate whether you're canceling the exam or requesting a late testing date.

Provide Additional Information for Late Testing Requests
If requesting a late testing date, explain the reason. The College Board requires this information to determine if a $40 late testing fee will apply.

After submission, you will receive an email with any changes or confirmations.
