Instrument Usage Form

In order to obtain a school instrument you need to:

1. Qualify for free or reduced lunch.

2. Turn in a copy of the letter from the district that states your family qualifies for free or reduced lunch to Mrs. Segapeli

3. Turn in a completed Application for School Owned Instrument Usage form. (See form below.)

4. $25 if your child has free lunch, $50 if your child has reduced lunch. Checks made out to Denton ISD.

EXCEPTIONS:

• Cello and Bass students who do not qualify for free or reduced lunch and who are already renting through a music company may rent a school instrument for $125 to use and keep at school. Please fill out Application for School Owned Instrument Usage form and make check out to Denton ISD.
