Former Student Transcript Request

Please complete the Former Student Transcript Request, also available in the Guyer Front Office, and return with a copy/picture of your ID and $2.00 processing fee per transcript, payable by cash, check, or money order via mail or in person, in the Guyer Main Front Office. Please allow 24-72 hours for processing, with more allowance for semester finals, summer hours, and/or spring/winter enrollments. 

*At this time there is not a way to pay electronically, by card, or over the phone. 

*At this time, transcripts can ONLY be mailed to the specific address listed in your request, or sent via Texas Records Exchange (TREx), which is available to most Texas public schools. 

*Transcripts can never be sent via email due to the confidential information they contain (i.e. full name, DOB, SSN).

NCAA

If you are interested in playing a sport when you attend college, you will need to register with the NCAA. The NCAA has created a website to assist all prospective athletes with the registration process HERE. During your senior year, you will need to request a certificate of amateurism. Athletic Recruitment Transcript Request

Collegiate recruiters can request via email for a specific student athlete, provided the student and parent (if the student is under 18) have signed THIS RELEASE. Email requests should include a secure fax line number. 

Diploma Reprinted Request (Very Rare)

If you are a Guyer graduate needing a replacement Diploma for an education verification for a new job or post-secondary education, more than nine times out of ten, an official transcript will more than suffice; a verification of graduation date that is all that's needed. If you have accidentally lost or destroyed your diploma, a reprint can be ordered for $45 and will take a minimum 10-12 weeks for printing and shipping. Please contact Mr. Travis Critchlow at Josten's at 940-382-8948 to reorder your diploma; pay by phone is available. 

Third Party Education Verification Requests

If you are a third party education verification service provider, please fax your request with an authorization signature (can be electronic) to 940.369.4964. Requests via phone or email, or without an authorization signature will not be accepted. Please allow 24-48 hours for processing.

Information regarding credits denied due to absences:

Texas Education Code (TEC 25.092) provides that “(a) Except as provided by this section, a student in any grade level from kindergarten through grade 12 may not be given credit or a final grade for a class unless the student is in attendance for at least 90 percent of the days the class is offered.”  All absences, whether excused or unexcused, must be considered in determining whether a student has attended the required percentage of days. 

In accordance with state statute, a student MAY not be awarded credit for a course if they have absences totaling more than 10% of class days for a course.  While reviewing your student’s report card, please notice the column indicating credits. If your student passed a class but received no credit, it could be related to excessive absences.

Per state law, parent notes do not excuse the 90% rule. However, state law allows provisions for regaining lost credit through building-level attendance review committees and administrative review. For further understanding, please see pages 26 and 31 of the 2019-2020  Student and Parent Handbook.  Please contact your student’s assistant principal if you have questions and/or concerns regarding credit recovery.
