PHYSICAL FORM

Please print off the form below for student physicals.  The questions on the left need to be completed as well as signatures at the bottom to validate the medical history.   Please print off and complete the two waivers below the physical (August Heart, Pre-Participation) if you are having the physicals performed on campus in May, and return to your coach or an athletic trainer by May 1, 2025.  The two waivers below are not needed if the physical is being performed on your own.
