Depending on which grade your child is in, they should proceed with different medical exams and vaccines. See resource section below for annual forms and additional vaccine information.

If you object to examination(s) or immunization(s) on religious grounds, please complete and submit this form. This must be signed by the parent/guardian and medical professional performing the examination in grades JK, SK, 6 and 12.
