The required WBL forms are provided below. These forms are fillable, so there is no need to print them; however, you will need to download DocHub in order to complete the form, create your signature, and sign the documents.

Parent/Guardian Permission and Emergency Packet
We want to make sure that everyone knows about and agrees with the steps you are about to take toward your career. Make sure this form is completed and signed. WBL internships cannot be completed without this form -- NO exceptions.
You will be expected to:
Show up on time and be prepared to work.
Sign in and out using the time sheet below.
Follow all policies and procedures for your worksite.
Have fun and get to know the people who are training you, but always remain professional.
If you are going to be late or absent, you must call and speak to your instructor and a supervisor at your worksite.
Please return your completed and signed time sheets every TWO weeks.
Each student will be evaluated after the first few weeks of an internship and also near the end.
You should know the areas in which you are doing well and the areas in which you need to improve.
A WBL Training Agreement will be given to your worksite supervisor to sign and complete.
At the end of your internship, you will then sign the form and return it to your instructor or Mrs. Hayek.
We try to place our students in businesses with safe, supportive, and educational environments, and student feedback is a critical part of the Work-Based Learning process.
Please complete and return the work site evaluation form to Mrs. Hayek via email at the end of your internship.
If you have an immediate concern about any Work-Based Learning experience, please do not wait to report that information.
