A form for submitting bypass requests on behalf of a participant. Prior training is required before submission and the link to the training can be found within the form itself. Please do not share this link with clients.
A form for providing regular ongoing check-ins or updates for CE enrolled clients that helps keep them active in our system. This form can be completed by both providers and the client themselves.
All service providers who are seeking access to the Thurston County Coordinated Entry RAGIC client database must complete the following agreement form.
Name by Name Meeting Participation Agreement:
A required agreement for any direct service providers that are wanting to attend the weekly Name by Name case conference meeting.
Veteran NBN Meeting Participation Agreement:
A required agreement for any direct Veteran service providers that are wanting to attend the weekly Veteran Name by Name case conference meeting.
