NEW STUDENTS:
We are so excited to have your student at Riddle! Please provide the school a current shot record from your doctor as soon as possible so we can complete the registration process for your child. We are not able to place your child in a classroom or assign a teacher until the shots are current and up-to-date. Immunization requirements and resources are provided below.
Diphtheria/Tetanus/Pertussis (DTap/DTP) - 4 or 5 doses; one dose must be received after 4th birthday
Polio - 3 or 4 doses; one dose must be received after 4th birthday
Measles, Mumps, and Rubella (MMR) - 2 doses; first dose must be received after 1st birthday
Hepatitis B - 3 doses
Varicella (Chickenpox) - 2 doses; first must be received after 1st birthday
Hepatitis A - 2 doses; first dose must be received after 1st birthday

State of Texas Immunization Requirements
Students may be exempt from having immunizations for the following reasons: Medically contra-indicated (must have a physician statement that is only good for 1 year), already had the disease (must have proof such as a titer drawn) and conscientious objection. This requires a separate form which the parent gets from the Department of State Health Services. It has to be notarized and is only good for two years from the date of notarization.
Exemption from Immunizations for Reasons of Conscience
Parents may request the above exemption form from the State of Texas.
Form must be completed and signed by parent/guardian
Form must be notarized
Form is good for two years from date of notarization
Forms will be mailed from Austin, TX, to parent/guardian; the student will be excluded from school until immunizations are current or a current exemption form is presented to the school (original copy).

The original copy must be retained in the campus clinic
NOTE: if the school experiences an outbreak of any of the diseases covered by immunizations, any student not fully immunized may be excluded from school for the health and safety of the student as well as other students.
