In accordance with new state law, every student participating in a club must submit a signed permission slip. 

Students: 

1.  Download the permission form

2.  Fill out the form and sign it using Kami or another PDF editor

3. Use this link to upload a PDF of the signed permission slip

You will only need to complete 1 permission slip.
